How do you ensure you are focused on learning?

I know this is a rather deep question, but I was just wondering how you make sure that when you're teaching you are focused on children's learning rather than just them doing the task. Is it always possible to do this?
I've been given a list of pointers - the main things observers are looking for, I know I do these things all the time, but not necessarily all in the same session. I know my children make progress over time, but find it hard to understand how to show progress in one lesson.

My last observation progress was seen by what they produced, my guided writing group and how they intereacted through an up level activity. Mixed with questioning to secure understanding.
